The upcoming November 27 PDUFA for Intermezzo presents two possibilites:

1. Intermezzo is rejected. I see this as a 60% likelihood. I was surprised by the FDA's decision to grant a two month review, but that is far from a guarantee of approval. I continue to maintain that Transcept's response does not satisfy the FDA's stated concerns in the CRL. If Intermezzo is approved, this will be the biggest shock since Vanda's Fanapt.

2. Intermezzo is approved. Needless to say, my score will immediately go 100-150 points into the red. But as with Vanda and Somaxon I believe the market will pick up the ball that the FDA dropped. Intermezzo is largely superfluous and their REMS will be unwieldy. The costs of marketing the drug will likely never converge with profits.

There you have it. Either immediate gratification or long-term fulfillment. I'm feeling good about this red thumb no matter what.

TSPT is likely headed for another CRL. Often, biotech companies prefer to take the cheap route, not performing the studies that the FDA would prefer to address questions raised in a previous CRL.
